Clear options.
Personalized care.
We accept and bill participating insurance plans. Self-pay options are available for Thrive and eligible Restore services.
How payment works for you.
Call with your plan information before your first visit. We’ll confirm participation, review benefits and requirements, and explain your expected costs.
| Your care or coverage | How payment works |
|---|---|
| Restore with in-network insurance | We bill your insurance directly. You pay any applicable deductible, copayment, or coinsurance. |
| Restore with Traditional Medicare | We bill Medicare directly. Your responsibility depends on your benefits and supplemental coverage. |
| Restore with Medicare Advantage | We confirm your plan’s benefits and requirements. We can bill directly when applicable coverage is available, including eligible out-of-network benefits. |
| Restore with out-of-network commercial insurance | You pay our self-pay rate. We can provide a superbill for possible reimbursement for eligible services. |
| Thrive | You pay privately at our listed self-pay rates. |
| Care without insurance | You pay our listed self-pay rates. We explain your service and cost before care begins. |
Payment is due at the time of service. This includes self-pay fees and any known copayment, deductible, or coinsurance. We will bill you for any additional patient balance determined after your insurance processes the claim.
Self-pay rates.
These rates apply to Thrive and eligible self-pay Restore services. Thrive offers sports performance, fitness, conditioning, and exercise guidance when skilled rehabilitation is not needed; its services are paid for privately.

For insurance-covered care, your plan’s benefits determine your share of the cost.
Details for your coverage.
Traditional Medicare.
OPTA bills Traditional Medicare for covered physical therapy services. We require a referral for skilled physical therapy through Restore.
Medicare Advantage.
We can bill your plan directly when applicable coverage is available, including eligible out-of-network benefits. Coverage, network, referral, and authorization requirements vary by plan.
Out-of-network commercial insurance.
For eligible Restore services, we can provide a superbill for you to submit for possible reimbursement. Check your out-of-network physical therapy benefits, deductible, referral or authorization requirements, and submission process. Reimbursement is not guaranteed. Thrive’s general fitness and conditioning services should not be assumed to qualify for physical therapy reimbursement.
Aetna.
We are currently out of network with Aetna. Commercial-plan patients may use self-pay and superbill options when appropriate. For Aetna Medicare Advantage, we can bill directly when applicable out-of-network benefits are available, subject to coverage and authorization requirements.
UnitedHealthcare: a change beginning January 1, 2027.
Effective January 1, 2027, OPTA will no longer participate as an in-network provider with UnitedHealthcare, including UMR and Golden Rule. This supports our commitment to individualized, one-on-one care.
For commercial plans, self-pay and superbill options will remain available when appropriate. For UnitedHealthcare Medicare Advantage, we can bill directly when applicable out-of-network benefits are available, subject to coverage and authorization requirements.
HSA and FSA payments.
