Choosing your care
What conditions do you treat, and is OPTA right for me?
OPTA helps with a wide range of conditions affecting movement, comfort, and daily function, as well as goals for sports performance and lifelong fitness.
Restore — Traditional physical therapy
We help you recover and regain function through care for:
- Joint and muscle pain: Back, neck, shoulder, elbow, wrist, hand, hip, knee, ankle, foot, and jaw/TMJ.
- Rehabilitation: Sports injuries, difficulty returning to activity, persistent pain, and recovery after orthopedic and spinal surgery.
- Vertigo, dizziness, and balance.
Thrive — Sports performance, conditioning, and wellness
When traditional physical therapy is not needed, we provide personalized exercise guidance for:
- Sports performance: Golf, running, tennis, pickleball, baseball, soccer, volleyball, lacrosse, swimming, biking, waterskiing, and football.
- Healthy aging and wellness: Building and maintaining strength, mobility, balance, and fitness for everyday life.
Since 2005, our Lakeland practice has focused on individualized, one-on-one care with experienced physical therapists. Your appointment is dedicated to you, with time to understand your concerns and develop a plan around your goals.
What is the difference between Restore and Thrive—and what if I’m unsure which I need?
Restore provides skilled physical therapy for pain, injury, surgery, or loss of function. Thrive provides personalized exercise and guidance for sports performance and to build and maintain strength, mobility, balance, and fitness when skilled rehabilitation is not needed.
Thrive visits may include guided exercise, activity-specific conditioning, and a program to follow between visits. You don’t need to choose a pathway before calling—we’ll help you find the appropriate starting point.
Can I start Thrive without having physical therapy first?
Yes. You can start Thrive without an injury or previous physical therapy. Whether you want to improve sports performance, prepare for a trip, build strength, or support healthy aging, we tailor your exercise and conditioning program to your goals.
Thrive also offers continued guidance after rehabilitation. Your initial assessment helps us determine whether Thrive fits your needs or whether traditional physical therapy would be more appropriate.
Do I need a doctor’s referral?
Restore — Traditional physical therapy
You may be able to begin care without a physician referral. Requirements depend on your service and insurance plan. Call our office before your first visit so we can explain any referral or authorization requirements.
For patients using Traditional Medicare, OPTA requires a referral for skilled physical therapy services through Restore.
Thrive — Sports performance, conditioning, and wellness
Thrive services do not require a doctor’s referral. You can contact us directly to schedule your Initial Assessment.
Do you treat vertigo and balance disorders?
Yes. OPTA evaluates and treats vertigo and balance disorders through our Restore pathway. If you experience spinning when rolling over in bed, looking up, or bending over, your therapist can assess whether positional vertigo (BPPV) may be contributing and provide appropriate treatment.
If you feel unsteady or worry about falling, we assess the factors affecting your balance. Your individualized plan may include strengthening, balance and walking exercises, and a home exercise program. If your findings suggest a need for medical evaluation, we’ll help guide you to appropriate care.
What is shockwave therapy, and what does it cost?
Shockwave therapy is a non-invasive treatment that applies acoustic waves to the affected area to help reduce pain and support tissue healing. It may be appropriate for certain persistent tendon and soft-tissue problems that have not improved with other treatments.
Treatment is applied directly to the skin and typically takes 5–10 minutes. Many patients feel improvement after 1–3 treatments, although results vary. A course often involves 3–6 sessions, usually about a week apart, depending on your condition and response. Your therapist will review your medical history and determine whether shockwave is appropriate for you.
Our current fee is $100 per treatment session. Call 863-644-0007 to learn more or discuss whether it may fit your needs.
Your visits
What happens at my first appointment, and what should I bring?
Your first visit is an Initial Assessment consisting of a 45-minute, one-on-one appointment focused on your goals and the right path forward. Your therapist will review your history and goals, perform a thorough physical assessment, and discuss recommendations with you.
Wear comfortable clothing that allows you to move easily. Bring your completed forms, any relevant medical information, and insurance information when applicable. Most forms are available under the Forms tab on our website to print, complete, and bring with you. Our office will confirm which forms you need and when to arrive.
How long are appointments, and will my therapist work one-on-one with me?
Initial appointments are 45 minutes, and follow-up visits are 30 minutes. Your scheduled appointment is dedicated one-on-one time with your physical therapist. We believe focused attention is essential to understanding your needs and helping you progress.
Will I see the same physical therapist at each visit?
Whenever possible, you will continue with the same therapist. This helps your therapist understand your history, track your progress, and adjust your plan as your needs change. If scheduling requires another therapist, our team will coordinate your care.
How often will I need to come, and for how long?
Your visit frequency and length of care depend on your needs, goals, and progress. For Restore, your therapist will recommend an appropriate rehabilitation plan and review it with you as you improve.
For Thrive, some people benefit from regular guided exercise, while others prefer periodic visits to review and progress an independent program. We’ll discuss an approach that fits your goals and schedule.
How do I schedule, and what if I need to cancel or reschedule?
Call 863-644-0007 to schedule an appointment. Our office team will help you choose the appropriate visit, review fees and coverage, and explain how to prepare.
We request 24 hours’ notice for cancellations or rescheduling so we may offer your appointment to another patient. A $50 cancellation fee may be charged if this notice is not provided.
Fees and insurance
How much do visits cost?
Our current self-pay rates are:
- Initial Assessment: $150 for 45 minutes
- Follow-up visit: $80 for 30 minutes
- Shockwave therapy: $100 per treatment session
Restore and Thrive currently use the same self-pay visit rates. For insurance-covered care, your responsibility depends on your benefits. We’ll review the applicable fees and payment process before care begins.
Which insurance plans do you accept, and what is changing with UnitedHealthcare?
OPTA continues to participate with selected insurance plans. When we are in network, we bill your insurance directly; your responsibility depends on your plan’s benefits. Call our office to confirm your specific plan.
We are currently out of network with Aetna. For commercial plans, self-pay and superbill options are available when appropriate.
Effective January 1, 2027, OPTA will no longer participate as an in-network provider with UnitedHealthcare, including UMR and Golden Rule. This change supports our commitment to individualized, one-on-one care. Self-pay and superbill options will remain available for eligible commercial-plan services.
If you have Aetna or UnitedHealthcare Medicare Advantage, contact us to confirm your benefits and direct-billing options. See the answers below for more about superbills and Medicare Advantage.
Can I receive care out of network and submit a superbill for reimbursement?
For eligible out-of-network physical therapy services, you may pay OPTA directly and request a superbill to submit to your insurance company. A superbill provides information about the services you received.
Your plan determines whether reimbursement is available and how deductibles, authorizations, and other requirements apply. Reimbursement is not guaranteed. Thrive’s general fitness and conditioning services should not be assumed to qualify for physical therapy reimbursement; our office can explain the documentation appropriate to your service.
Do you accept Traditional Medicare and Medicare Advantage plans?
OPTA provides covered physical therapy through Traditional Medicare and follows applicable billing requirements.
Medicare Advantage plans have their own networks and coverage rules. Please contact us with your specific plan information before scheduling. If your plan includes applicable out-of-network benefits, we can bill your Medicare Advantage plan directly even when we are out of network, subject to the plan’s coverage and authorization requirements.
Thrive is intended for fitness, conditioning, and ongoing exercise guidance when skilled rehabilitation is not needed. Our team will help distinguish the appropriate service and payment process. The pathway name alone does not determine insurance coverage.

Your care can change with them.
